Pass entitles the cardholder to one (1) session at 50% OFF with each participating Practitioner/Therapist including: Chiropractor Holistic Healthcare Coach Nutritionist Homeopathy/Naturopath Bodywork/Massage Reflexologist *Call for pricing and availability Pass must have paid receipt attached to be valid. Pass may not be photocopied, sold, or transferred. Pass valid for 1 year from date on receipt. Cannot be combined with any other new patient/client specials at participating Practitioners/Therapists.
Membership  $49/year

PRACTITIONER/THERAPIST
Full payment for yearly membership is due at time of sign-up. Practitioner/Therapist will be listed in six (6) consecutive bi-monthly print issues of CREATIONS MAGAZINE and posting on creationsmagazine.com for 12 consecutive months, with a link to Practitioner/Therapist’s website. Practitioner/Therapist honors one (1) session at 50% OFF per valid Pass (per membership year). Practitioner/Therapist is responsible for checking validity of Pass.
